Polish for exLOC integration

Additions

• Tests:
  • analyze JSON includes lines + meta.lineCountMode and verifies executable < physical when comments present
  • ratchet FAIL-case prints Lines + Assessment (executable up/down, docs up/down)
• Ratchet: show Lines section on OK and FAIL; add brief assessment on FAIL
• Docs: EXECUTABLE-LINES.md with config example; CONTEXT-AWARE-LINTING.md includes sample Lines output
• ESLint: ignore prototype sandbox and disable guardrails for scripts/** (tooling)

Why

• Validates the telemetry wiring and improves UX without changing gating

Validation

• Tests: 601 passing, 3 pending
• Ratchet: OK — no increases; Lines printed (informational)
