I'm also experiencing this issue. A partial workaround I found is that if you switch to a different model for one request and back to GPT-5, it will usually work for two or three requests before the problem occurs again.

I click "Retry," sometimes multiple times, and it works. This happens randomly, sometimes at the beginning of the prompt, and sometimes while Copilot is running. But only when GPT-5 is selected.
