The bash tool should return a notification that the process timed out.

The bash tool returns nothing if it times out. It seems that it should be possible to return a message that the bash tool timed out so the LLM know specifically and doesn't assume that its command syntax was bad.

@bhauman can you point to where this should be changed? AFAICS at src/clojure_mcp/tools/bash/core.clj already handles timeout on both nrepl bash or local bash.
